A patient’s question

How many grafts you need and how it is worked out

The short answer: the exact number is given after the examination, while a photo gives an estimate — and that is enough to understand the order of magnitude and put a plan together. What is counted is not “how much hair you would like”, but how much can be taken from the donor area and how much is needed to cover the area with natural density. These are two different figures, and the smaller one decides the result.

What the number depends on

Size of the area

The larger the area, the more grafts — but not in direct proportion: the hairline and temples need a different density from the crown.

Stage of hair loss

A receding hairline with temple recession is one task, a thinning crown is another, and they are counted separately.

The donor area

Hair is taken from the back and sides of the head. How much is there and how dense it is sets the ceiling you cannot go above. That is exactly what the specialist looks at.

The density you want

Dense placement along the hairline looks more natural, but uses more grafts for the same area.

How we assess it

Photos

You send the area in ordinary light, from the front, from above and from behind — the donor area only shows in the last shot.

Preliminary estimate

The specialist gives the order of magnitude and the suitable technique — Sapphire FUE or DHI.

Examination on site

The donor area is examined in person and its density is counted. This is where the estimate becomes a number.

The plan

The number of grafts, the technique and the timing are fixed before the procedure.

What a photo cannot show

The real density of the donor area

In a photo it looks denser than it is. It can only be counted in person.

The condition of the scalp

Inflammation, seborrhoea or scars from previous surgery change both the plan and the number.

The cause of hair loss

If hair keeps falling out, a transplant will cover the area but will not stop the process. That is a conversation for before the procedure, not after.

How many will take

Graft survival depends on aftercare and on the body. An honest estimate is a range, not an exact number.

Common questions

It depends on the area and the donor zone. Large volumes are sometimes split into two sessions with a break, so that the donor area can recover. The specialist decides this at the examination, not the wish to finish sooner.
A graft is the unit of transplantation, and it holds from one to four hairs. So “3,000 grafts” and “3,000 hairs” are different things, and offers should be compared in grafts.
Hair is taken from it selectively, and with the right calculation it does not visibly thin. That is exactly why the graft ceiling exists and is not exceeded.
